Join us on Friday, September 30, for "Public Health INsights and INnovations," a free online training series presented by the Center for Public Health Practice and the Indiana Public Health Training Center at the IU Richard M. Fairbanks School of Public Health. Over the coming months we'll use the Robert Wood Johnson Foundation's Culture of Health Action Framework to explore how hospitals, health departments, and non-profit organizations can work together to achieve our shared goal of improving health in our communities.
This month, we'll begin a three part series on Action Area 4 of the framework: "Strengthening Integration of Health Services and Systems." Raquel Castro, ASN, of HealthVisions Midwest will lead our first presentation. She will share her experiences identifying access issues for disparate populations and how to implement evidence based practices to improve access. You'll have an opportunity to share your own experiences and insights during the Q & A following the discussion.

Public Health Success Story
The Vanderburgh County Health Department’s Switch2Health coalition was selected as Indiana’s population health success story by the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA), Region V Great Lakes Public Health Training Collaborative (PHTC). Switch2Health demonstrated excellence by significantly increasing vaccination rates through the integration of public health and primary care. Using the Institute of Medicine’s framework for improving population health, their work on community collaboration, shared use of data, aligned leadership, and sustained systems integration has been a great success!
